Network in Aging presents a “Night Under the Big Top” Networking Event

 Buffalo, NY (June 26, 2018) – This month’s networking event for health care professionals will feature a circus theme and is to be held on Tuesday, July 31 from 4 pm to 7 pm at Buffalo Center for Nursing and Rehabilitation, located at 1014 Delaware Avenue.  Guests will enjoy access to a beer and wine tent, favorite carnival snacks, authentic entertainment and a midway of interactive games. Tours of the facilities recently renovated building, including brand new therapy gym will also be available. This event is sponsored by Centers Health Care, Fallon Health Weinberg – PACE and Eden Heights.

Once a month, select member agencies of Network in Aging collaborate to develop a themed “Network with the Network” event.  These complimentary events are designed for members to meet regularly in a variety of settings and are also open to area professionals who want to learn more about the sponsoring organizations and the Network.

About Fallon Health Weinberg-PACE

Fallon Health Weinberg-PACE is a long-term care program for older adults who need a nursing-home level of care, but wish to stay living at home and in their community. We are a Program of All-inclusive Care for the Elderly (PACE), providing complete medical care and support services so you or your loved one can remain living safely at home.   All care is coordinated and provided by an interdisciplinary team specializing in geriatric care, who work with participants and their caregivers to create an individualized care plan.  To be eligible to join Fallon Health Weinberg-PACE you must: be at least 55 years old, live in Erie County, NY, be certified as needing nursing facility level of care as determined by the NYS assessment tool, Require Fallon Health Weinberg-PACE long-term care services for more than 120 days, be able to live safely in the community, as defined by the New York State Health and Safety Criteria and the PACE Care Team, be Medicaid and/or Medicare eligible or willing to pay privately, and meet other eligibility requirements.
